MMDモデル利用報告フォーム
ニコニコ動画のコンテンツツリー以外では、作者へのフィードバックを行うのが難しいので、
そこらへんを簡単・気軽にしてもらえるようにフィードバック用フォーム作ってみました。
基本的に事後報告用です。
公開や利用前に相談したい事などありましたら、お問い合わせフォームやtwitterアカウントにお願いします。
